Join a physician-owned practice caring for adults 18 and older in our Novi office and for older, medically complex patients in homes and senior communities. This is longitudinal primary care: you become a trusted clinician for patients and families, rather than completing one-time assessments.

Flexibility with continuity: Our client considers home-focused, office-focused and blended schedules, and aim to accommodate your strengths and preferences. Your primary setting, usual workdays, field territory and expected cross-coverage are discussed before hire. The role is primarily weekdays, with shared telephone coverage and no acute-care hospital rounding.

Clinical breadth and support: Combine adult primary care with geriatrics, dementia care, behavioral health and geriatric behavioral medicine, chronic pain management, and palliative and hospice coordination. Use individualized medication optimization, including careful reduction of controlled substances when clinically appropriate. Provide prevention, acute assessment, chronic disease management and goals-of-care conversations, with timely escalation when a patient needs a higher level of care. Work directly with the physician-owner for case consultation and mentorship, supported by clinical and administrative teammates and CGM APRIMA training.

Compensation and benefits: A guaranteed minimum with uncapped productivity opportunity rewards effective, clinically appropriate work. The advertised range is a target for full-time total cash compensation—not a guaranteed base salary or a promise of earnings. The minimum, productivity formula, paid onboarding and schedule assumptions are explained during recruitment and documented in the written offer. Medical, dental and vision coverage, retirement plan participation, paid time off and holidays are available to eligible employees under plan terms. Professional liability coverage, CME/licensure support and a field vehicle allowance are included under the applicable employment terms.

Responsibilities & Duties:
What you bring: Michigan RN licensure with NP specialty certification and national certification appropriate to adult primary care, or Michigan PA licensure and NCCPA certification; eligibility considered. Required prescribing authority and payer participation must be in place for assigned work. Experience in primary care, geriatrics, home-based care, home health, hospice or skilled nursing is valued. Exceptional early-career clinicians may be considered with a defined mentorship plan. We look for sound judgment, respectful teamwork, accurate documentation and reliable follow-through. Field work requires a valid driver’s license and reliable insured transportation. APRIMA experience is a plus, not a prerequisite. Physician-led and professionally engaged.
