Salary: RN: $34/hr-$42/hr | LPN: $25/hr-$31/hr

Responsibilities matched to your license:
  • RN assignments include nursing assessment, protocol-based telephone triage, care planning and escalation. 
  • LPN assignments emphasize information gathering, implementation of care plans, patient education and follow-up under required supervision. 
  • The responsibilities and support arrangements are defined according to license, experience and demonstrated competency; the two licenses are not treated as interchangeable.

The work:
  • Coordinate transitions after hospital or facility discharge, medication-list review, chronic care follow-up and behavioral health care coordination. 
  • Track results, referrals, refill requests, authorizations and orders through resolution. 
  • Escalate urgent concerns promptly and maintain a reliable follow-up queue for routine items. 
  • Support program consent, accurate documentation and time tracking when required. 
  • The purpose is appropriate care and complete follow-through—not merely completing a monthly call.

Office and field opportunities:
  • Office-focused, field-focused and blended assignments are considered. 
  • Clinical tasks may include patient education, immunizations, specimen collection, wound care or home-safety observations within the agreed role, orders and required supervision. 
  • Primarily weekday work; any telephone coverage outside scheduled hours, schedule expectations and compensation are discussed before hire. 
  • Field work requires a valid driver’s license and reliable insured transportation; a vehicle allowance applies under policy.

Responsibilities & Duties:
What you bring:
  • Active Michigan RN or LPN license and current BLS. 
  • Primary care, home health, hospice, geriatrics, skilled nursing, triage or care-management experience is valued.
  • Two years of relevant nursing experience is preferred. 
  • Clear communication, accurate documentation, sound escalation judgment and respect for patients with dementia and their caregivers are essential. 
  • APRIMA experience is helpful; training is provided.
